As a public agency, the Department of Behavioral Health and Developmental Disabilities (BHDD) Office of Mental Health maintains public records in accordance with the South Carolina Freedom of Information Act. Medical Records
- Medical records are not public documents and are therefore not subject to the Freedom of Information Act. If you are seeking copies of your treatment records, please follow the instructions on our Medical Records Request page. This page provides the form you will need and the addresses and phone numbers of all BHDD Office of Mental Health facilities.
Employment Records
- If you are an employee or former employee seeking a copy of your employment records, a Freedom of Information Act is not required. Please contact BHDD Office of Mental Health Human Resource Services at (803) 898-8607 to make a request.
- If you are a member of the public seeking employee information, a Freedom of Information Act request is required. Please note that not all employee information is public information and BHDD Office of Mental Health will make redactions in accordance with South Carolina’s Freedom of Information Act statute, section 30-4-40.

Office of Substance Use Services
The business of the BHDD Office of Substance Use Services is open to public review, as is required by the state’s Freedom of Information Act (South Carolina Code Section 30-4). Requests may be submitted in writing to OSUS' General Counsel at: foia-request@osus.bhdd.sc.gov.
